Enrollment Requirements

We need the following items before a child can be enrolled at Cornerstone Learning Center


  • Application Form
  • Registration & Book Fees
  • Child’s Birth Certificate
  • Child’s Social Security Number
  • Medical Treatment Consent Form (Notarized)
  • Updated Immunization Records
  • Emergency Phone Contact List
  • Signed Tennessee Licensing Requirements
  • Signed Parent Handbook Acknowledgment
  • A Pre-Enrollment Visit

  • Tuition Contract
  • Application Fee
  • Book & Technology Fees
  • Child’s Social Security Number
  • Immunization Records
  • Birth Certificate
  • Signed Parent Handbook Acknowledgment
  • Teacher Recommendation
  • A pre-enrollment visit is required of all potential new students.
  • Admissions testing or copy of most current report card.

Any changes to phone numbers, addresses, or emergency contacts must be reported immediately to the office so we may update your child’s records.
